The successful provider will need to be able to communicate and liaise with our population through the medium of Welsh and provide in-person home contacts to the hard-to-reach areas across the rural geography due to limited public transport options, and patientsâ inability to travel independently, therefore, this would be a critical factor for the service provision. The service will deliver a bereavement and anticipatory grief service to adults aged over 18 years to support a patients bereavement recovery and adjust to life after loss via various mediums including but not limited to, telephone, virtually via a suitable online platform and in person.
